The following release is required to complete background checks when the discussions between a candidate and client enter the final stages. Background checks will be done only if both parties are serious about entering an employment relationship.

PERSONAL INFORMATION RELEASE AUTHORIZATION AND CONSENT

 I understand that Phoenix Technical Services, Inc. (PTS) is in the business of placing qualified candidates with its clients and that some PTS clients have specifically requested that employees undergo one or more background investigations and reference checks. In addition they may request information contained in PTS' personnel file of the employee.

I understand that PTS may use outside, independent or third-party agencies such as Consumer Reporting Agencies (CRA) to perform these background investigations on PTS' behalf.

I hereby release employers, schools, colleges, universities and other persons from all liability in responding to enquiries connected with my application and I specifically authorize the release of information by any of the schools, colleges, universities, individuals or other entities provided by me to PTS or its Clients, either in writing, email or verbally.

By signing this form, I hereby authorize and consent to PTS conducting such investigations and obtaining such reports with respect to me, and sharing the results of such investigation as well as information contained in my personnel file, with the requesting Client.

I have read and understand the above. By signing this Authorization and Consent, I release PTS, the requesting client, their respective parent, subsidiary and affiliate companies and directors, officers and employees from any and all liability relative to such background investigation and/ or the use of the results of such background investigation in making employment-related decisions.
